Services Provided

Reset Mobile Phlebotomy Services LLC provides mobile specimen collection services including, but not limited to:

• Routine and specialty blood collections 
• Therapeutic phlebotomy 
• Drug and alcohol testing 
• DNA testing specimen collection 
• Specialty laboratory kit collections 
• Medical courier services  
• Other mobile specimen collection services as offered 

We do not diagnose, interpret laboratory results, prescribe treatment, or provide medical advice. Laboratory results are provided directly to the ordering healthcare provider or authorized laboratory. 

Appointment Scheduling

Appointments are subject to availability and confirmation.

Clients are responsible for providing accurate information when scheduling services. Failure to provide accurate information may result in delays, rescheduling, or cancellation of services.

Payment Policy

Payment is due at the time of booking unless alternative arrangements have been approved.

Accepted payment methods are identified during the scheduling process.

Services may be canceled or delayed if payment has not been received.

Cancellation and Rescheduling

Clients must provide a minimum of 24 hours' notice for cancellations or rescheduling requests.

Appointments canceled with less than 24 hours' notice may be subject to a cancellation fee.

No-show appointments may result in the assessment of a no-show fee equal to the reserved service fee and applicable travel charges.

Travel Fees

Additional travel fees may apply based on service location and mileage outside of our standard service area. Any applicable travel fees will be disclosed before services are rendered.

Specimen Collection Requirements

Clients must:

• Present valid identification when required 
• Provide laboratory orders when applicable 
• Follow pre-test instructions provided by their healthcare provider or laboratory 

Reset Mobile Phlebotomy Services LLC is not responsible for testing delays or recollection requirements resulting from inaccurate information, incomplete orders, patient non-compliance, or laboratory requirements.

Limitation of Liability

While we strive to provide professional and accurate services, Reset Mobile Phlebotomy Services LLC shall not be liable for:

• Delays caused by laboratories, healthcare providers, weather, traffic, or unforeseen circumstances 
• Laboratory testing errors 
• Physician interpretation of results 
• Indirect, incidental, or consequential damages arising from use of our services
